Appointment cancellation and “no show” Policy:

We require that our patients cancel at least 24 hours prior to the given appointment. We prefer even longer prior notice if possible. Patients who either fail to show for their appointment or cancel in less than 24 hours prior to the appointment will be charged a cancellation fee of $40 payable before the next appointment.

Billing Policy:

All charges including co-pays that are not billable to insurances are payable at the time of service. We do not routinely bill patients for services that are due at the time of services and a billing charge of $5 will be applied if payment is not made at the time of service.

Most insurances cover acupuncture. If your insurance does not cover acupuncture, you will be expected to pay at the time of service. Treatments such as many types of injection therapies and manipulation therapies may or may not be covered. These therapies are charges that are in addition to the acupuncture. In those instances that these services are covered, our office will bill your insurance directly: if not then the patient will need to pay for those non-covered services

Insurance

Insurance coverage is a contract between you and your insurance company. Fiquet Hanna Duckworth, DOM, PC is not a party to this contract. All Insurance policies have a deductible for the year and then a co-payment and/or co-insurance. To find out what your policy terms and coverage are, call your insurance company. The number is on the back of your insurance card.
